Application

Once the qualifications for Board Qualified Status are met, the first step is to request an initial application. This starts the 7 year timeline for case accumulation.


Once the qualifications for Board Certification have been met, the next step is to request an application for Certification.

Please request either Board Qualified or Board Certified applications via email to admin@abpsx.org If you need help in completing the forms, please feel free to contact us.

There are 2 types of Certifications depending on the type and complexity of surgical cases you choose to present for review.


1) Diplomate, American Board of Podiatry Surgeons-Foot and Ankle Surgery
2) Diplomate, American Board of Podiatry Surgeons-Advanced Ankle and Rearfoot Reconstruction

 

All applications must contain the following enclosures:

  • Graduation from an accredited medical School

  • Completed lower extremity residency containing surgical training. (PSR, PMSR for DPM’s Orthopedic Surgical Residency with Lower Extremity Fellowship for MD’s and DO’s)

  • Copies of your current state license(s) to practice and current DEA license.

  • Proof of current malpractice insurance
